A $25 non-refundable registation fee is required with this family registration form.   This
registration fee enables any family member to participate in any activity at Vinings Academy for
which they have registered & paid any corresponding fees. All participants must abide by the
rules & requirements for each program.

Refund Policy:   10% of class fees are non-refundable, the balance of the class fees are
refundable up to 3 weeks before the start of classes. After that time, no refunds will be given.
Students may be allowed to change to another program based on program and space availability.
In the event that a class or workshop is cancelled, either a refund or a make-up credit will be
issued at the discretion of Vinings Academy management.

Medical Authorization : In case of an emergency on school property involving my
child,_______________________    which in the opinion of the school personnel present,
requires immediate medical or surgical attention, I hereby grant permission to said personnel to
obtain the sevice of a physician or Emergency Medical Technician for treatment and/or transport
of my child to the hospital if it is deemed necessary.

Media Release: I give permission for photos & recordings (video & sound) of my child taken
during class times to be used by Vinings Academy. in promotional materials and on their website.
Prior to any usage parents will be contacted with the specfic photo or recording to be used.
Children will never be identified by name without specific written permission in addition to this
authorization.

Early Drop-off / Late Pick-up: Students may not be dropped off prior to class beginning parents
are welcome to come in with their student prior to class start but we request that you are aware of
other classes that may be going on and make every effort not to create distractions. All students
must be picked up within 5 minutes of class release.   Late fees of $1/minute will be charged for
children not picked up within this time.

Attendence: Because of the academic nature of our programs we request that students make
every effort to attend the clubs in which they are enrolled. Make-up classes are NOT available.   I
understand that non-attendence does not free me from any financial commitment that I have
made with Vinings Academy.

Returned Payments: In the event that any payment is returned by my bank, I understand that I
will need to resubmit my payment or reauthorize my credit card payment   I will also pay an
additional $29 as a returned payment fee.
